Clinical Course of Nontuberculous Mycobacterial Pulmonary Disease

Abstract

|
Purpose: Nontuberculous mycobacteria (NTM) pulmonary disease is frequently misdiagnosed as tuberculosis, and it is important to distinguish the two diseases for proper management. The number of the disease has been recently increased, but there is seldom report of clinical manifestations of the disease in Chungbuk.
Materials and Methods: NTM pulmonary disease patient who visited Chungbuk National University Hospital since 2004 were enrolled and their medical records were reviewed retrospectively.
Results: There were 30 patients whose repiratory specimen showed NTM culture positive. Fifteen patients (50%) had NTM pulmonary disease. MAC was the most frequent organism (in 11 patients, 73.3%) and M.abscessus (in 2 patients 13.3%) and M.kansasii (in 1 patient, 0.9%) was the next in order. MAC pulmonary disease showed nodular bronchiectatic form in 5 (45.5%) patients and fibrocavitary form in 6(54.5%) patients. M.abscessus pulmonary disease showed nodular bronchiectatic form in both 2 (100%) patients. Six (60%) MAC pulmonary disease patients had successful treatment with negative conversion sputum, but 4 (40%) MAC pulmonary disease patients failed to reach negative conversion. Two M.abscessus pulmonary disease patients failed to reach negative conversion. One M.kansasii pulmonary disease patient is still under the treatment.
Conclusion: Half of the patients with NTM culture positive respiratory specimen had NTM pulmonary disease. MAC was the most frequent organism, and M.abscessus and M.kansasii was the next in order. Sixty percent patients of MAC pulmonary disease were successfully treated, and both M.abscessus pulmonary disease patients failed to treat.
